Brow Lamination Consent Form

Come ready so your lift sets cleanly:

Arrive with clean brows — no makeup or oils on the area.
Pause retinol and exfoliating acids near the brows for a few days before.
If you’ve never had a lamination or have sensitive skin, let us know so we can patch test.

For the first 24 hours — this part matters most:

Keep brows completely dry — no water, steam, or sweat for 24 hours (the lift is still setting).
No makeup or products on the brows for 24 hours.
Brush them into shape with a clean spoolie once they’re set.
Avoid steam rooms, saunas, and swimming for 24 hours.
Your lamination lasts about 6–8 weeks — brush daily to keep it neat.

Please check any conditions that apply to you. If any box is checked, your technician will review before proceeding. If a tint is added, a patch test is required 24–48 hours beforehand.

Known allergy or sensitivity to lamination / perm / keratin solutions Broken, irritated, or sunburned skin in the brow area Eczema, dermatitis, or psoriasis in the brow area Brow lamination, tint, or henna within the last 4–6 weeks Accutane/isotretinoin or retinoids within the last 6 months Recent chemical peel, waxing, or microdermabrasion in the area (within 2 weeks) Very sparse, damaged, or over-processed brow hairs Pregnant or breastfeeding

I understand brow lamination is semi-permanent (typically lasting 4–6 weeks), that results vary with my natural hair, and that I must keep brows dry and avoid steam, makeup, and rubbing for 24–48 hours after service.
